Med-Peds Continuity Clinic

Our Internal Medicine and Pediatrics Primary Care Practice is located in the Jackson Memorial Hospital Ambulatory Care Center (ACC). The site houses both our faculty and resident practices and is the University of Miami and Jackson Memorial Hospital transitional care referral site for adolescents and young adults with special health care needs.

Our primary care practice focuses on providing a medical home for our patients to ensure high quality care that is accessible, comprehensive and continuous for all patients regardless of insurance status.

The resident practice is run in a private practice team model.  All patients are scheduled under the resident’s name and each intern is linked to the panel of one of our graduating seniors at the beginning of the academic year based on individual strengths and attributes and with attention to cultural and linguistic competency.  Residents are responsible for all aspects of care for their patient panel including guiding core measure quality improvement projects. Residents function in 5 separate teams, with team members providing coverage for follow-ups and patient communication during vacations or difficult rotations.

All ambulatory practices at JMH use Cerner®, our electronic medical record system, for scheduling, documentation, orders and prescriptions.  Patients are referred by our inpatient normal and intensive care nurseries, adult and pediatric inpatient wards, emergency rooms and subspecialty practices, which allows resident retention of patients to ensure continuity of care.
