Transition Medicine Track
Our newest career track is the Transition Medicine Track. This program is designed to develop leaders in the field of Transition Medicine providers and educators. This program is run by Dr. Jonathan Tolentino and Dr. Anjali Saxena.
As part of this track, residents will complete the transition medicine elective during their PGY3 or PGY4 year focused on didactics, resources, and clinical experiences to enhance the understanding of the transition process. In addition, residents will participate in monthly transition medicine journal clubs and participate in the LEND Educational series as well. All resident will complete a capstone project focused on a transition research or implementation project here at the University of Miami or Jackson. Residents will also actively participation in the Lifespan group at UM focused on improving the care of developmentally disabled South Floridians and to support and build new programs by partnering with local community-based organizations. Residents are also highly encouraged to participate in the American Academy of Developmental Medicine and Dentistry (AADMD), SGIM ACCOC Interest Group, or other similar organization. Active participation in any local (hospital, university, or program), regional, or national committees or organizations or groups.

In addition, residents will have the opportunity to participate in our new PATH (Progression Across Transition in Healthcare) Program. Our goal at PATH is to create a transitional care network of providers for this intellectually disabled and developmentally disabled (ID/DD) population. This network will provide a coordinated, interdisciplinary program leveraging the use of telehealth to move outside the traditional clinic, allowing for flexibility for the patients and the providers to access care through the network and collaborate in an interdisciplinary manner to provide comprehensive care. As part of the PATH Program, residents will have the opportunity to work in the PATH clinic, focused on providing coordinated care for patients with developmental disabilities transitioning from pediatric to adult care. This clinic is staffed by Dr. Jonathan Tolentino, MD, and Dr. Anjali Saxena, and our care coordinator Shelly Baer, MSW.
